1. Dapoxetine

What It Is:

Dapoxetine is a short-acting selective serotonin reuptake inhibitor (SSRI) developed specifically for the treatment of premature ejaculation (PE) in men.

Mechanism of Action:
  • Inhibits the serotonin transporter (SERT), increasing serotonin’s action at the synaptic cleft.
  • Enhances the latency period before ejaculation by modulating the central nervous system.
Uses:
  • Primary indication: Premature ejaculation (PE).
  • Often taken on-demand, 1–3 hours before sexual activity.
Benefits:
  • Increases Intravaginal Ejaculatory Latency Time (IELT).
  • Improves control over ejaculation.
  • Enhances sexual satisfaction and relationship confidence.
Risks / Side Effects:

2. Sildenafil Citrate (brand name: Viagros)
What It Is:

Mechanism of Action:
  • Inhibits phosphodiesterase type 5 (PDE5).
  • Increases cGMP levels, causing vasodilation in the corpus cavernosum of the penis, resulting in improved erectile response to sexual stimulation.
Uses:
  • Erectile dysfunction (ED).
  • Pulmonary arterial hypertension (under brand Revatio).
Benefits:
  • Effective in producing and maintaining erections.

What It Is:

Mechanism of Action:
  • Inhibits PDE5, allowing smooth muscle relaxation and increased blood flow in the penis.
  • Unlike sildenafil, it also has activity in the prostate and bladder (hence use in BPH).
Uses:
  • Erectile dysfunction (ED).
  • Benign prostatic hyperplasia (BPH).
  • Pulmonary arterial hypertension.
Benefits:
  • Long half-life (17.5 hours) → effects can last up to 36 hours ("weekend pill").
  • Available as daily low-dose or on-demand.
  • Improves urinary symptoms in BPH.
Risks / Side Effects:
  • Common: Headache, back pain, muscle aches, flushing, indigestion.
  • Serious: Same as sildenafil (priapism, hearing/vision loss).

  • Combination Use Considerations:
  • Sometimes, Dapoxetine + Sildenafil or Dapoxetine + Tadalafil are combined for men experiencing both ED and PE.

Risks of Combination:
  • Higher risk of hypotension.
