What It Is:
Dapoxetine is a short-acting selective serotonin reuptake inhibitor (SSRI) developed specifically for the treatment of premature ejaculation (PE) in men.

- Increases Intravaginal Ejaculatory Latency Time (IELT).
- Improves control over ejaculation.
- Enhances sexual satisfaction and relationship confidence.
- Common: Nausea, dizziness, headache, diarrhea, insomnia.
- Serious: Syncope (fainting), mood changes, serotonin syndrome (rare).
- Contraindicated in: Significant heart disease, severe liver impairment, and with certain psychiatric medications (e.g., MAOIs, SSRIs).

- Pulmonary arterial hypertension (under brand Revatio).
- Effective in producing and maintaining erections.
- Rapid onset (30–60 minutes), lasts ~4–6 hours.
- Improves sexual confidence and quality of life.
- Common: Headache, flushing, nasal congestion, dyspepsia, visual disturbances (blue-tinted vision).
- Serious: Hypotension (especially with nitrates), priapism (erection lasting >4 hours), sudden hearing or vision loss.
- Contraindicated in: Use of nitrates, recent stroke or MI, severe hypotension.

Mechanism of Action:
- Inhibits PDE5, allowing smooth muscle relaxation and increased blood flow in the penis.
- Unlike sildenafil, it also has activity in the prostate and bladder (hence use in BPH).
- Erectile dysfunction (ED).
- Benign prostatic hyperplasia (BPH).
- Pulmonary arterial hypertension.
- Long half-life (17.5 hours) → effects can last up to 36 hours ("weekend pill").
- Available as daily low-dose or on-demand.
- Improves urinary symptoms in BPH.
- Common: Headache, back pain, muscle aches, flushing, indigestion.
- Serious: Same as sildenafil (priapism, hearing/vision loss).
- Less likely to cause visual changes than sildenafil.
- Contraindicated in: Use of nitrates, severe cardiac conditions.
- Combination Use Considerations:
- Sometimes, Dapoxetine + Sildenafil or Dapoxetine + Tadalafil are combined for men experiencing both ED and PE.

Risks of Combination:
- Higher risk of hypotension.
- Increased likelihood of side effects like dizziness, headache.
